Introduction to the case
Sometimes the decay is so deep it is difficult to get a matrix down to properly seal the gingival margin. What I have done several times is use a matrix inside another matrix just for the gingival margin.
New patient had a deep cavity in a RC tooth. Decay was removed without rubber dam and some gingival tissue going in the box was removed as well.
Matrix band in Tofflemire with another smaller matrix inside that has been pushed beyond the gingival margin.
If you magnify the picture you will see the smaller band easily.
Small matrix cut to fit so it will slide beyond gingival margin inside larger matrix band.
This is a new uncut Premier Cure Thru Matrix band. The distal section of the band was left but the mesial was trimmed shorter since the mesial didn’t need to go so far gingvally. These matrix bands are excellent for deep decay but sometimes you need to reach further gingivally as in this case.
The nice thing about this situation was the adjacent tooth needed a restoration so we use this technique easily. Clear matrix helps with curing since you can cure thru the band.
Deep restoration is easily seen on post op X-ray.
